The foundation's Racial and Ethnic Health Care Equity focus area emphasizes “promoting equity in health and health care for common chronic conditions and infant mortality,” while the Integrated Health Care program supports “advancing high-quality health care by improving care coordination and communications among health care professionals; creating informed and involved patients; and promoting cost-effective, affordable care.”
Research, project, and policy grants may be awarded.
